S-adenosyl-L-methionine (SAMe) is a naturally occurring biomolecule present in all living cells that serves as an essential coenzyme in various metabolic pathways. It is synthesized in the cellular cytoplasm from the essential amino acid methionine and adenosine triphosphate (ATP), a reaction catalyzed by the enzyme methionine adenosyltransferase (MAT).

SAMe features a high-energy, positively charged sulfonium bond that destabilizes its attached methyl group, making it highly reactive and easily transferable. This unique chemical structure allows SAMe to readily donate methyl groups to DNA, proteins, and lipids via methyltransferase enzymes, driving vast epigenetic and structural modifications.

In transmethylation, SAMe acts as the universal methyl donor for various substrates, converting itself into S-adenosylhomocysteine (SAH) in the process. In transsulfuration, homocysteine derived from SAMe is enzymatically converted into cysteine, which is the rate-limiting precursor for the critical antioxidant glutathione. In the aminopropylation pathway, decarboxylated SAMe provides the aminopropyl groups required to synthesize polyamines (spermine and spermidine), which regulate cell growth and survival.

SAMe supplementation is frequently used to support liver function, particularly in conditions like intrahepatic cholestasis, by replenishing depleted hepatic SAMe reserves. By stimulating the transsulfuration pathway, it enhances the endogenous synthesis of glutathione, restoring the liver's primary antioxidant defenses and protecting hepatocytes from oxidative damage.

For depression, SAMe is thought to improve mood by increasing central nervous system methylation, which accelerates the synthesis and turnover of critical neurotransmitters like serotonin, dopamine, and norepinephrine. In joint health, clinical studies suggest SAMe can reduce osteoarthritis pain and stiffness as effectively as some NSAIDs, likely by stimulating proteoglycan synthesis and exerting mild anti-inflammatory effects.

While SAMe is the direct, active methyl donor utilized by cellular enzymes, folate and vitamin B12 act as essential upstream cofactors required to continuously recycle SAMe. Specifically, folate supplies the raw methyl group and vitamin B12 facilitates its transfer to homocysteine, successfully remanufacturing methionine so it can be converted back into active SAMe.

SAMe is generally well-tolerated, with the most common mild side effects being gastrointestinal distress, nausea, and insomnia, particularly at higher doses. However, it is strictly contraindicated in individuals with bipolar disorder due to the risk of triggering manic episodes, and must be used with extreme caution alongside SSRI antidepressants to prevent serotonin syndrome.
