1,3-Di-o-tolylguanidine (DTG) is a synthetic substituted guanidine derivative widely used as a research compound. It acts as a non-selective agonist at both sigma-1 and sigma-2 receptors throughout the central nervous system and peripheral tissues. It exhibits high selectivity for sigma binding sites over traditional opioid, adrenergic, and dopaminergic receptors.

DTG functions as a full agonist at both sigma receptor subtypes, initiating downstream intracellular cascades upon binding. At Sigma-1 receptors, DTG activation modulates intracellular calcium fluxes, stimulates protein kinase C pathways, and potentiates NMDA receptor signaling. At Sigma-2 receptors, DTG activation influences intracellular calcium levels, cellular viability, and can trigger apoptotic signaling pathways in proliferating cells.

DTG produces antidepressant-like behavioral effects in animal models, such as significantly reducing immobility in the forced swim test, particularly when combined with NMDA receptor modulators. Its neuroprotective properties stem from its ability to prevent intracellular calcium dysregulation during ischemic stress and attenuate glutamate-induced excitotoxicity. Additionally, DTG activation of sigma pathways helps preserve neuronal survival and modulate neuroinflammatory responses.

DTG displays balanced, low-nanomolar affinity for both Sigma-1 (Ki = 35-69 nM) and Sigma-2 (Ki = 21-40 nM) receptors. Unlike subtype-selective ligands that favor one subtype by orders of magnitude, DTG binds both Sigma-1 and Sigma-2 receptors with roughly equal potency. This non-selective, equal-affinity profile sets DTG apart from most selective sigma receptor ligands.

While DTG is a pan-sigma receptor agonist that binds Sigma-1 and Sigma-2 receptors equally, (+)-pentazocine is a prototypical, highly selective Sigma-1 receptor agonist. Because (+)-pentazocine displays very low affinity for Sigma-2 receptors, it is routinely used alongside or in competition with DTG to differentiate or isolate subtype-specific biological responses.

Radiolabeled 3H-DTG is widely used as a standard radioligand in binding assays to label total sigma receptor populations in cell and tissue membrane preparations. When (+)-pentazocine is added to the assay buffer to mask Sigma-1 sites, 3H-DTG serves as a classic tool for specifically measuring and screening Sigma-2 receptor binding parameters. It also acts as a reference ligand for mapping sigma receptor distribution in central nervous system structures.

Although DTG itself is a laboratory tool rather than an approved therapeutic, its identification established sigma receptors as distinct pharmacological targets. It served as a baseline chemical template for medicinal chemists to synthesize refined, subtype-selective guanidine and heterocyclic derivatives. These DTG-inspired structural classes have guided the ongoing development of selective Sigma-1 and Sigma-2 ligands for cancer therapeutics, neuropathic pain, and neurodegenerative disorders.
