[H-3] Cytisine

Cytisine is a plant-derived alkaloid naturally found in seeds of the Cytisus laburnum tree that has been used in Central and Eastern Europe for decades as a smoking cessation aid. It acts as a high-affinity, selective ligand for central nicotinic acetylcholine receptors (nAChRs) in the brain. By binding to these receptors, it competitive blocks nicotine from binding while simultaneously triggering mild receptor activation.

As a partial agonist at α4β2 subtype nAChRs, cytisine binds with high affinity but produces only a fraction of the full intrinsic signal triggered by nicotine or acetylcholine. This partial activation releases low, sustained levels of dopamine to attenuate nicotine withdrawal symptoms and cravings. At the same time, its high binding affinity prevents inhaled nicotine from binding, effectively blocking the rewarding sensation of smoking.

Varenicline is a synthetic drug designed based on the chemical structure of cytisine, possessing higher oral bioavailability and a significantly longer terminal half-life (approx. 24 hours vs. approx. 4.8 hours). Consequently, cytisine requires a frequent daily dosing schedule that tapers over a short 25-day treatment period, whereas varenicline is taken twice daily over a standard 12-week course. Despite these logistical differences, both medications share the same mechanism of action as partial agonists at α4β2 nicotinic receptors and demonstrate comparable efficacy for smoking cessation.

While cytisine functions as a partial agonist at α4β2 receptors, it acts as a high-potency full agonist at α3β4 nicotinic acetylcholine receptor subtypes. These α3β4 receptors are predominantly localized in the autonomic ganglia and adrenal medulla. Full activation of this subtype accounts for many of cytisine's peripheral ganglionic and cardiovascular responses.

Cytisine is primarily used as an effective, cost-efficient oral pharmacotherapy to aid smoking cessation and treat nicotine dependence. By binding to central nicotinic receptors, it alleviates nicotine withdrawal symptoms and reduces cravings during abstinence. Additionally, its competitive receptor binding attenuates the rewarding effects if a person smokes while taking the medication.

Common side effects are generally mild and gastrointestinal in nature, including nausea, dry mouth, dyspepsia, and abdominal discomfort, along with sleep disturbances like insomnia. Because of its full agonist activity at peripheral autonomic α3β4 receptors, it can cause transient elevations in heart rate and blood pressure, requiring cautious use in patients with unstable cardiovascular disease. High overdoses carry a risk of severe nicotinic toxicity, presenting with vomiting, muscle weakness, respiratory depression, and convulsions.

Cytisine is a naturally occurring quinolizidine alkaloid found in several plant genera within the Fabaceae (legume) family. It is particularly concentrated in the seeds, leaves, and bark of the Golden Rain tree (Laburnum anagyroides) and Laburnum alpinum. It also occurs in other plant species such as Cytisus, Baptisia (wild indigo), and Genista (broom).
