[H-3] Nitrendipine

Nitrendipine is a synthetic, second-generation dihydropyridine derivative used as a cardiovascular medication. It belongs to the dihydropyridine class of calcium channel blockers (CCBs) and functions primarily as a selective peripheral vasodilator.

Nitrendipine selectively binds to voltage-gated L-type calcium channels located in vascular smooth muscle cells, inhibiting the influx of extracellular calcium ions. This blockade prevents calcium-dependent muscle contraction, leading to arterial vasodilation, a significant reduction in systemic vascular resistance, and a subsequent drop in blood pressure.

Nitrendipine is primarily indicated for the management of essential (primary) hypertension, ranging from mild to moderate stages. It is also utilized in elderly hypertensive populations and can serve as a long-term therapy to reduce the risk of hypertension-associated cardiovascular and cerebrovascular complications.

While short-acting nifedipine requires sustained-release formulations or frequent dosing due to its rapid elimination, nitrendipine has a longer duration of action and a smoother pharmacokinetic profile. Compared to amlodipine—a third-generation CCB with an ultra-long elimination half-life (approx. 30-50 hours)—nitrendipine possesses a shorter half-life (approx. 8-12 hours) and typically requires once- or twice-daily dosing.

Common side effects stem directly from its vasodilatory actions and include peripheral edema (ankle swelling), facial flushing, headaches, and dizziness. Less frequently, patients may experience reflex tachycardia, palpitations, gastrointestinal disturbances, or fatigue.

Nitrendipine is effective both as a first-line monotherapy for mild-to-moderate hypertension and as part of a combination regimen for resistant or severe cases. It works synergistically when combined with other antihypertensive classes, such as ACE inhibitors, angiotensin receptor blockers (ARBs), or thiazide diuretics.

Nitrendipine's clinical efficacy is strongly supported by major clinical trials, most notably the landmark Syst-Eur (Systolic Hypertension in Europe) trial. In this study, nitrendipine-based therapy demonstrated significant reductions in overall stroke incidence, cardiovascular events, and dementia development in elderly patients with isolated systolic hypertension.
