GR 125743 is a synthetic benzamide and pyridine derivative that acts as a potent, competitive serotonin receptor antagonist. It specifically targets and blocks serotonin 5-HT1B and 5-HT1D receptor subtypes across central and peripheral nervous systems.

GR 125743 selectively antagonizes 5-HT1B and 5-HT1D receptors by binding directly to their orthosteric ligand-binding domains, preventing endogenous serotonin (5-HT) or synthetic agonists like sumatriptan from initiating G-protein signaling cascades. It exhibits significantly higher selectivity and affinity for these two subtypes compared to other serotonergic, adrenergic, or dopaminergic receptor families.

For wild-type human cloned receptors, GR 125743 exhibits a pKi value of approximately 8.85 (= 1.4 nM) at h5-HT1B receptors. At wild-type h5-HT1D receptors, it binds with a slightly lower, yet high-affinity pKi value of 8.31 (= 4.9 nM).

While both are research compounds developed by Glaxo (now GSK), they target completely distinct serotonin receptor families. GR 125743 is a 5-HT1B/1D receptor antagonist, whereas GR 113808 is a highly selective antagonist for the 5-HT4 receptor subtype.

In preclinical neuroscience, GR 125743 is primarily used to investigate 5-HT1B/1D autoreceptor and heteroreceptor modulation of neurotransmitter release, cortical circuitry, and cardiovascular functions. Furthermore, its radiolabeled variant ([3H]-GR 125743) is extensively employed in radioligand binding assays and autoradiography to map 5-HT1B/1D receptor density in brain tissue.

GR 125743 itself remains an experimental laboratory tool rather than an approved therapeutic. However, pharmacological research into 5-HT1B/1D receptor ligands paved the way for clinically successful antimigraine medications, such as triptans (e.g., sumatriptan), which exploit the same receptor targets as agonists to promote cranial vasoconstriction and inhibit neurogenic inflammation.
