[H-3] Thyrotropin-releasing hormone

Thyrotropin-releasing hormone (TRH) is a hypothalamic neurohormone that acts as the primary master regulator of the hypothalamic-pituitary-thyroid axis. It is predominantly synthesized by parvocellular neurons in the paraventricular nucleus of the hypothalamus and secreted into the hypophyseal portal network. Outside the hypothalamus, smaller quantities of TRH are also produced throughout other brain regions, the gastrointestinal tract, and the pancreas.

TRH binds to G-protein-coupled receptors on pituitary thyrotrophs, triggering the phospholipase C signaling pathway. This cascade rapidly elevates intracellular calcium, which drives the exocytosis of stored TSH vesicles into the bloodstream while upregulating new TSH gene expression. Circulating thyroid hormones T3 and T4 exert negative feedback on this mechanism by downregulating both TRH production and pituitary receptor sensitivity.

TRH is a tripeptide with the primary amino acid sequence L-pyroglutamyl-L-histidyl-L-prolineamide (pGlu-His-Pro-NH2). Its chemical structure features key modifications at both ends: a cyclized N-terminus forming a pyroglutamic ring and an amidated C-terminus on the proline residue. These distinct chemical modifications are crucial for its biological activity and afford it protection against immediate enzymatic cleavage.

TRH has a remarkably short half-life in peripheral circulation, typically lasting only 2 to 6 minutes. It is rapidly cleaved and inactivated by specific circulating and membrane-bound enzymes, primarily TRH-degrading ectoenzyme (pyroglutamyl peptidase II). This rapid clearance ensures that hypothalamic signaling remains precise, localized, and capable of quick adjustments via negative feedback.

Beyond regulating thyroid hormones, TRH acts directly on anterior pituitary lactotropes to stimulate prolactin synthesis and release. Within the central nervous system, TRH functions as a widespread neurotransmitter and neuromodulator that regulates arousal, thermogenesis, autonomic tone, and energy balance. It also modulates limbic pathways, influencing mood stabilization and pain regulation.

Historically, the TRH stimulation test evaluated thyroid and pituitary responsiveness by administering synthetic TRH intravenously and measuring subsequent TSH levels. It helped clinicians differentiate between primary, secondary, and tertiary hypothyroidism, as well as confirm borderline hyperthyroidism.

Thyrotropin-releasing hormone (TRH) directly influences both prolactin secretion and energy balance. It acts on the anterior pituitary to stimulate prolactin release alongside thyroid-stimulating hormone (TSH), making TRH a potent physiological secretagogue for prolactin. Simultaneously, TRH regulates energy balance centrally by promoting satiety and peripherally by driving thyroid hormone production, which elevates basal metabolic rate.
